Communion & Visit Request

If you would like to request Holy Communion to be brought to a family member, we ask you to please submit the information by filling out the form on the right. You could also use this form to request a visit from our pastor, Fr. Truc.

The person submitting the form must be an immediate family member or an authorized caregiver. The person for whom the request is being made must be willing to receive Holy Communion.

Please be aware the a staff from the parish office will follow up on this petition and will be put on a list for Ministers' review. Appointments with Fr. Truc are based on availability. Thank you and God bless.
